Worked through as a series of scenarios:

The workForm
A new circuit added to the installationEIC
Adding a socket to an existing circuitMinor Works (an EIC would also be acceptable)
Consumer unit changeEIC
Upgrading a 7 kW shower to 9.5 kW, needing a bigger cableEIC — the circuit's cable is replaced, and the testing changes
Replacing a circuit's cable with a different typeEIC — the circuit is rewired
Like-for-like replacement — 2.5 mm² twin and earth for the sameMinor Works
Like-for-like RCD replacementMinor Works
Which form for which job the question is whether there is a new circuit, a new board, or a rewired circuit EIC Electrical Installation Certificate a new circuit a consumer unit change 7 kW shower to 9.5 kW — bigger cable a circuit's cable replaced with another type a new or rewired circuit, or a new board Minor Works no new circuit adding a socket to an existing circuit like-for-like 2.5 mm² twin and earth like-for-like RCD replacement no new circuit, or a like-for-like swap Schedules circuit details and test results — always, alongside whichever certificate you used EICR — not one of these a condition report, not a certificate you do not fill one in as part of initial verification — it belongs to periodic inspection
The test is simple: does the work include a new circuit, a new consumer unit, or a circuit's cable replaced? Then it takes an EIC — which is why a shower upgrade needing a bigger cable is an EIC and not Minor Works. Anything less, such as a socket added to an existing circuit or a like-for-like swap, is Minor Works. The schedules go with either. And the EICR is not yours here: it is a report on an existing installation, not a certificate for work you have done.
The question to ask is whether a new circuit is involved — or a new consumer unit, or a circuit's cable replaced. If not, an addition or alteration to an existing circuit, or a straight swap of the same thing for the same thing, is Minor Works.
