One per distribution board. A shed sub-board fed from the house board gets its own, and its “supplied from” is the house board, not the mains.

The top of the sheet: the board

Board reference and location, what it’s supplied from, the overcurrent device feeding it, the SPD type (T1, T2, T3 or N/A), and the RCD details if the board has one protecting everything.

One row per circuit

ColumnWhat goes in it
Circuit number and descriptionEvery way on the board. Spares get a line drawn through, so nobody can add to the sheet later
Type of wiringA code from the key at the bottom of the form (below)
Reference methodFrom BS 7671 Table 4A2: e.g. C clipped direct, B in conduit or trunking on a wall, 100–103 for twin and earth run through thermal insulation in ceilings or stud walls
Number of points servedThe points of utilization on the circuit: sockets, luminaires, outlets
Live and CPC csae.g. 2.5 / 1.5 for a ring in twin and earth
Overcurrent deviceBS (EN) number, type, rating, and breaking capacity in kA
Maximum permitted ZsFrom the BS 7671 tables: B32 is 1.37 Ω, B6 is 7.28 Ω
RCDBS (EN) number, type, IΔn and rating — or N/A if the circuit has none
Codes for type of wiring
CodeWiring
AThermoplastic insulated and sheathed cables (twin and earth)
BThermoplastic cables in metallic conduit
CThermoplastic cables in non-metallic conduit
DThermoplastic cables in metallic trunking
EThermoplastic cables in non-metallic trunking
FThermoplastic SWA
GThermosetting SWA
HMineral insulated
OOther — say what

A circuit run in more than one wiring system gets more than one code.

✋If your max Zs doesn’t come from the BS 7671 tables, say so. You can use the corrected values from Guidance Note 3 or the On-Site Guide, but then you have to state the source in the Remarks column (column 31) of the schedule of test results. The footnote on the form says so. The simplest approach is to take the tabulated value from BS 7671.
Read the device, not the paperwork. The BS (EN) number, type, rating and breaking capacity are printed on the front or side of every breaker and RCD. The breaking capacity is the number in the rectangle: 6000 means you write 6, because the column is already in kA. Check it yourself rather than taking the installer’s word, because counterfeit devices exist.

A Type 3 SPD fitted to protect sensitive equipment goes in the Remarks column.
